Volunteer Forms

Thank you for your interest in volunteering with CIES and Channel Islands National Park! Your safety is very important to us, and so prior to coming out on a volunteer trip with CIES you will need to read through informational documents, watch safety videos and fill out volunteer forms. Please follow the directions carefully and send completed volunteer services agreement form and hantavirus acknowledgment waiver to:

*Forms need to arrive and be on file at least 2 days in advance of your scheduled trip, so please plan accordingly.


 

Volunteer Services Agreement

You will be signed up as a National Park Service volunteer when you come out with CIES. Please fill out each highlighted area designated on the Volunteer Services Agreement form below, print out, check an appropriate box, sign on the second page, and mail it to the address above. We need the hard copy, wet ink signature of your form. No electronic volunteer services agreement forms are accepted by Channel Islands National Park.

 

Hantavirus Acknowledgement Form

Read through the Notice of Hantavirus Presence in Channel Islands National Park document and then print and sign the Hantavirus Acknowledgement Waiver linked below and mail it to the address above. We need the hard copy, wet ink signature of your form. No electronic Hantavirus forms are accepted by Channel Islands National Park.

 

CINP Boat Transportation Safety Information and Self Assessment Form

Traveling aboard the Channel Islands National Park Service Vessels is a wonderful experience for our volunteers. They are true ‘working vessels’ carrying staff and materials to and from the 5 national park islands each week. There are safety considerations and physical requirements that must be taken into consideration before signing up for a volunteer trip aboard these vessels in order to maintain the safety of all of our volunteers.

Please do the following:

  • Read the boat transportation guide
  • Watch the boat safety videos, and
  • Submit the online self assessment form linked below before your scheduled volunteer day. For the online sef assessment, input david_mazurkiewicz@nps.gov as your Supervisor/Park point of contact. He is the seabird biologist and Channel Islands National Park liaison for CIES. Mark your affiliation as volunteer.
